refactor(mcp): migrate to high-level server API

Summary

• replace the deprecated low-level Server API with McpServer
• register MCP tools through registerTool instead of manual request handlers
• pass Zod schemas directly to McpServer for schema generation and input validation
• preserve the existing MCP tool names, descriptions, annotations, and stdio transport

Testing

• eslint --fix (via the pre-commit lint-staged hook)
• git diff --check
• Not run: pnpm build and pnpm test (project validation commands are restricted in the current environment)
