Summary

The token endpoint answers every failure with a plain-text body (http.Error), and reports an upstream refresh rejection as 502 Token refresh failed. MCP clients parse token endpoint errors as JSON per RFC 6749 §5.2, so on a plain-text 502 they can't tell an expired grant from a transient outage. They keep retrying the refresh instead of starting a new authorization.

Seen in production with Entra ID: a conditional access sign-in frequency policy rejected the refresh token (AADSTS70043, invalid_grant), and Claude Code retried the refresh
~2,300 times over ~19 hours without ever prompting the user to sign in again.

Changes

  • HandleToken now returns {"error": ..., "error_description": ...} with Content-Type: application/json (plus the existing security headers, including Cache-Control: no-store) from every OAuth error path.
  • Upstream invalid_grant, invalid_request, invalid_scope and unauthorized_client (read from oauth2.RetrieveError) are relayed as 400 with the provider's code and
    description, so clients re-authorize.
  • Other upstream failures (invalid_client, 5xx, network errors) keep their 500/502 status, now as server_error. These reflect the proxy's configuration or the provider's
    availability, not the client's grant.
  • Local validation failures use invalid_request (malformed body, oversized or missing parameters) and unsupported_grant_type.
  • Non-OAuth responses are unchanged (native-mode 404, 405).

Testing

  • Tests pass locally (go test ./..., go vet, golangci-lint: 0 issues)
  • New tests added (TestHandleTokenErrorResponses): upstream invalid_grant on refresh and code exchange, upstream invalid_client and 503, missing refresh token,
unsupported grant type, oversized parameter. Each asserts status, JSON error, Content-Type and no-store, against a fake token endpoint.

Summary by CodeRabbit

  • Bug Fixes
    • Token endpoint errors now use OAuth 2.0 JSON responses instead of plain text for malformed requests, missing credentials, and unsupported grant types.
    • Recognized upstream token errors are returned with their error code and description. Other upstream failures use a consistent server_error response, with status codes reflecting the token flow.
    • Error responses include a JSON content type and no-store caching headers to prevent caching sensitive error details.

HandleToken answered every failure with a plain-text body via
http.Error, and reported an upstream refresh rejection as
502 "Token refresh failed". MCP clients parse token endpoint errors as
JSON per RFC 6749 section 5.2; on a plain-text 502 they cannot tell an
expired grant from a transient outage, so they keep retrying the refresh
instead of starting a new authorization. With Entra ID, a conditional
access sign-in frequency policy (AADSTS70043) produced thousands of
retries over many hours and never a sign-in prompt.

Return `{"error": ..., "error_description": ...}` with
Content-Type application/json from every OAuth error path:

- Upstream invalid_grant, invalid_request, invalid_scope and
  unauthorized_client (from oauth2.RetrieveError) are relayed as 400
with the provider's code and description, so clients re-authorize.
- Other upstream failures (invalid_client, 5xx, network) stay 500/502,
now as server_error; they reflect the proxy, not the client's grant.
- Local validation failures use invalid_request and
  unsupported_grant_type.

Walkthrough

HandleToken returns OAuth-style JSON errors for malformed requests, unsupported grants, and upstream token failures. Responses include security headers. Tests cover response status, error fields, content type, and caching.

Security architecture risk: 🔵 Low · up to 3b759

The new errors should help clients distinguish expired grants from outages. The main security consideration is that selected provider descriptions are now returned to token callers. No credential disclosure is demonstrated.

Retained concerns

  • Low · security · observed: Selected upstream error descriptions now pass unfiltered from the provider to token endpoint callers. The supplied evidence does not establish that those descriptions contain secrets or private data, but the change broadens the diagnostic information exposed across that boundary.
Security review details

Security Blast Radius

  • inferred — The changed disclosure is limited to error responses from the existing token endpoint in proxy mode. A caller must submit a grant that produces an eligible upstream error to receive its description.

Security Findings and Attack Paths

  • inferred — A caller can obtain provider-supplied diagnostic text for an eligible failed exchange. The supplied example is an expired-grant message; the evidence does not show a secret or private-data leak.

Trust Boundaries and Controls

  • observed — The provider-to-client relay requires an OAuth retrieval error, an upstream 4xx status, and an allowlisted code. Other failures use a fixed description; JSON encoding and no-store headers constrain response handling but do not redact relayed text.

Hardening Proposals

  • proposed — Consider retaining the actionable upstream error code while filtering or replacing provider descriptions if supported providers can include account-specific or sensitive diagnostics.

Comment thread handlers.go Resolved
golang.org/x/oauth2 parses the error code from any non-2xx token
response, so a 5xx whose body carries "invalid_grant" was relayed as a
400. Clients would then treat a transient provider failure as an expired
grant and force a new sign-in. Relay the provider's code only when the
upstream status is 4xx; otherwise keep the 500/502 server_error fallback.

Add coverage for the three advertised upstream error codes.

token_errors_test.go tests upstream relaying only for invalid_grant. Local invalid_request cases do not exercise writeUpstreamTokenError. A regression that removes invalid_request, invalid_scope, or unauthorized_client from relayedTokenErrors can therefore pass the current tests and return server_error instead of the upstream code.

Add upstream-relaying test cases to the token error table for invalid_request,
invalid_scope, and unauthorized_client. Each should use a refresh-token request
with a 400 upstream response and assert that the matching upstream error code
and status are returned through writeUpstreamTokenError.

114-129: 🎯 Functional Correctness | 🔵 Trivial | ⚡ Quick win

Add a missing authorization-code test case.

The fixture sends an oversized authorization_code value, but it does not send grant_type=authorization_code without code. A regression in that branch from JSON invalid_request back to the previous plain-text response would therefore pass the changed tests. Add a case that omits code and asserts the JSON error.
